How it works
Early death claims occur in any life or funeral book, and most are genuine. A cluster is different: several early claims linked by the same intermediary, area, certifying doctor, funeral home or beneficiary, within a period too short to explain by chance.
Clusters usually point to lives insured while already terminally ill with the illness withheld, or to policies taken on people who had already died, supported by fraudulent or backdated death certificates. In rare and serious cases the insured was killed for the benefit. The certifying doctor, funeral home and intermediary may be part of the arrangement, or may simply be used repeatedly by the people behind it.
Each claim arrives with a death certificate, a valid policy and a beneficiary with an insurable interest, so file-level review finds little to question. The signal is in the distribution: when early claims are grouped by intermediary, branch, area, doctor or funeral home, a few groups carry far more than their share.
Red flags
- Death within the first months of cover, or shortly after the waiting period ends
- Same doctor certifying natural deaths across several early claims, with no hospital record
- Same funeral home, intermediary or beneficiary appearing on several early claims
- Medical history showing a diagnosis that predates the proposal
- Cover increased, or new lives added, shortly before the death
- Death away from home or in another region, with few corroborating records
Signals the engine evaluates
- Early-claim rate by intermediary, branch, area, certifying doctor and funeral home, against per-segment baselines
- Time from inception, and from the end of the waiting period, to date of death, compared across the book
- Shared beneficiaries, payer accounts, phones and addresses linking early claims on the network graph
- Concentration of deaths certified by one doctor across unrelated early claims
Investigation and response
- 01Hold payment on claims in the cluster while the link is investigated, within the turnaround times regulation requires
- 02Obtain medical records, hospital admission history and the original death notification, and verify them with the issuer
- 03Interview the certifying doctor, funeral home and beneficiaries, and confirm burial or cremation details
- 04Refer the cluster to SIU and, where the cause of death is in doubt, to law enforcement
